Hospitals: a new way to manage discharges and discharges for ambulatory major surgery and childbirth

Reduction of unnecessary trips

Hospitals have implemented a new measure that allows them to directly manage discharges and discharges for ambulatory major surgery and childbirth. This initiative aims to reduce users’ unnecessary trips to primary care centers (CAP). According to Department of Health estimates, this new way of managing discharges and discharges could avoid nearly 100,000 annual trips for users to CAPs, allowing health staff to spend more time caring for patients.

Expansion of the service throughout Catalonia

This new form of management will be added to work leaves due to scheduled or urgent hospital admissions that are already processed by all SISCAT hospitals. It is expected that hospitals will gradually start offering this service to users throughout Catalonia from next July.

Major ambulatory surgery

In the case of major ambulatory surgery (MA), which is a major surgical intervention that does not require hospital admission, it is estimated that more than 97% of patients in an active working situation could request a leave of absence from work according to the associated diagnosis. Of this percentage, approximately 10% will require a leave of less than five days, and in these cases the hospital center will process the leave and the discharge in the same administrative act. As for the other patients, the hospital center will only process the discharge and subsequent follow-up will be carried out from primary care. It is estimated that the management of CMA discharges from hospital centers will save between 64,000 and 85,000 user trips to CAPs.

Regarding childbirth, the new procedure that will be activated from hospitals is the discharge for women who already had an active leave of absence at the time of childbirth. It is estimated that approximately 41% of women who enter the hospital to give birth are in this situation. With this new management, nearly 17,000 annual trips to CAPs to manage medical discharge could be avoided.

Debureaucratization of primary care

The management of sick leave from hospital centers is part of the debureaucratization of primary care. This initiative, together with the other cases of sick leave that are already processed from the SISCAT hospitals, will allow to avoid an average of 200,000 unnecessary journeys for users throughout the territory each year (https://www.softcatala.org/resum- of-texts-in-catalan/).

Pilot testing and progressive implementation

The new way of managing discharges and discharges for major outpatient surgery and childbirth has already been tested as a pilot test at the Parc Taulí Hospital in Sabadell, and from this March users can request the processing of this type of unemployment on a regular basis.
